Information on SSM Claw Foot Tub

I have a SSM Tub and I can't find the correct faucet. It has 3 holes, one is the drain, on is about 4" above the drain and the other is near the top of the tub. I'm not sure what the holes are for or what the faucet looks like. I would also like the date if anyone knows this.The marking on the bottom is as follows:
